Appointment Requests

We are not accepting new patients at this time. Established patients should submit appointment requests via their child’s patient portal by following the link below.

Insurance

Your health insurance provider determines whether our clinic is in or out of network for your plan. It is ultimately the patient’s responsibility to pay for deductibles, co-payments, co-insurance or any other office charges incurred that are not covered by supplemental insurance at the time of service. For questions regarding your specific coverage and benefits, please consult your health insurance provider.

Note: Lactation appointments are scheduled for both mother and infant. Instead of being an out-of-pocket expense for families, each patient’s insurance is separately billed for this visit, however insurance co-pays or deductibles will still apply for each patient.

Patient forms

In an effort to reduce unnecessary paper use, all required office forms are located in your HIPAA compliant patient portal. These forms can be completed via your smartphone or tablet in the convenience of your home. Please ensure your child is registered with our Patient Portal and all documents are completed prior to your arrival. We regret that we are unable to see patients who have not completed their required medical paperwork prior to their visit, but will gladly help you to reschedule quickly.

Office policies

Castle Pines Pediatrics believes that vaccines play a vital role in protecting your child’s health. Our practice follows the CDC and AAP Guidelines for childhood immunizations. Dr. James and Dr. Mulhausen vaccinate their own children on time, every time, because vaccines save lives.

Any family refusing vaccines for non-medical reasons should seek medical care at another clinic better aligned with your immunization philosophy.

We understand the unexpected can keep you from attending a scheduled appointment. Please notify us as soon as possible, and we will work to reschedule your visit at a mutually convenient time.

We encourage you to reserve your child’s next check-up during your current visit in order to select the day and time that work best for your schedule.

Failure to appear at your scheduled appointment time without advance notice will incur a $50 no-show fee. No-show fees are not covered by insurance companies and will be the responsibility of the patient. Patients who no-show 2 or more times in a year are invited to establish care with another medical home.

Despite our best efforts, sometimes medical procedures cannot be completed as planned. We understand that it may be frustrating and inconvenient to have to schedule another visit, but please know we have your child’s best interest at heart. We ask that you respect our policy to not restrain or otherwise cause distress to a child in order to complete non-emergent medical work.
